Once you’ve booked your first appointment, please follow the steps below to make your first appointment as smooth as possible:
- Please fill out our New Patient Form & email it to our office before your visit to save time during your complimentary consultation. You may access & email it HERE.
- An hour before your appointment, we’ll send you a COVID questionnaire via text to make sure everyone is as safe as possible.
- Once you arrive, you’ll find our best parking located at the rear of the building. You may access this by the alley behind Hattie B’s.
Please call or text us at (615-472-1800) when you arrive & we’ll greet you at your car. Afterwards, we’ll take your temperature & give you a tour of the place!
