Am I covered?
If you have dental insurance, there’s a possibility you may have orthodontic coverage. To find out if you’re covered, contact your insurance provider and ask the following questions:
- Does my policy cover orthodontic treatment?
- What is the lifetime maximum?
- Has any amount been used already?
- Is there a deductible?
- Is there a waiting period?
- Is there an age limit?
- Is orthodontic work-in-progress covered?
- Do standard coordination of benefits apply if there is more than one policy?

We verify your benefits as a courtesy
Our team will verify your estimated insurance with the provider given to us. This is to help our patients plan financially and does not guarantee payment. As the patient, you are responsible for any payments that insurance does not cover. We ask that you realize we don’t work for an insurance company, but we do work 100% for our patients. Most insurance companies provide great benefits for our patients and we do everything we can to maximize your benefits.
How does insurance pay for my coverage?
Typically, insurance companies pay their portion of coverage over time and not in one lump sum. When paying out, this often looks like an initial down payment, followed by monthly, quarterly, or yearly payments. Before changing or canceling any insurance policies, please call your insurance provider to check your balance to make an educated decision on how to move forward. If any changes do occur, please contact our office via phone, text, or email as soon as possible to make adjustments.
Your responsibility As a Patient
If for some reason estimated insurance benefits are not received, the responsible party will assume the unpaid balance. We provide in-house payment plans to best help you achieve your goals. At your initial consultation, a treatment plan will be provided with several financial options to help achieve the care you need. Please understand that the fees we charge and the treatment Dr. Foltz recommends is specifically designed for your individual needs and never based on your insurance coverage.
